Position Summary

The Enhanced Care Management (ECM) Supervisor is responsible for overseeing the day-to-day operations of the ECM program and ensuring high-quality service delivery. This role provides supervision and support to ECM staff, including case managers, outreach specialists, and engagement specialists while serving as a key liaison between front line staff and program management. The ECM Supervisor monitors staff performance, facilitates communication, ensures compliance with program requirements, and supports the achievement of organizational goals and quality standards.

Essential Duties & Responsibilities

  • Supervise ECM staff to ensure program goals, timelines, and quality standards are consistently met, with emphasis on data monitoring, effective communication, and cross department collaboration.
  • Partner with the ECM Manager to plan, implement, and evaluate staff training and education focused on core services, reporting requirements, and quality improvement initiatives.
  • Oversee and provide guidance on documentation within the Electronic Health Record (EHR), including G-Codes and Modifiers, ensuring compliance with regulatory and program standards.
  • Serve as a key point of contact during Kern Health System audits; assist with documentation, reporting, and corrective actions as needed.
  • Compile, review, and submit data for grant applications and reports, ensuring accuracy and timeliness.
  • Collaborate with Billing, IT, and Kern Health Systems to ensure accurate data exchange and identify opportunities for process improvement.
  • Monitor and support ECM billing processes to ensure timely and accurate claim submissions.
  • Provide the ECM Manager with updates on staff performance, program trends, and challenges affecting patient care quality.
  • Monitor patient satisfaction within ECM services and contribute to quality improvement initiatives based on feedback.
  • Support ECM Case Managers with complex cases and, when necessary, participate directly in clinical or field encounters to ensure continuity of care.
  • Track staff productivity and compliance, ensuring adherence to program requirements and departmental objectives.
  • Assist with the delivery of ECM services when needed, maintaining program responsiveness to patient needs.
  • Promote and support the adoption of best practices in care management, fostering staff engagement in process improvement initiatives.
  • Manage day-to-day administrative functions, including recordkeeping, paperwork processing, and program coordination.
  • Complete and assist with annual performance evaluations for ECM staff in collaboration with the ECM Manager.
  • Foster a positive, inclusive work environment and encourage teamwork and cohesion across ECM teams.
  • Perform other duties as assigned.


Requirements:

Education & Experience Requirements

  • Bachelor’s degree in Social Work, Psychology, Public Health, or a related field required.
  • Supervisory experience preferred.
  • Experience in program implementation and evaluation within healthcare or community health settings, particularly serving vulnerable populations.

Skills & Knowledge

  • Proficiency in Microsoft Excel and Word; strong computer literacy.
  • Excellent written and verbal communication skills.
  • Strong ability to delegate tasks and provide effective support to a multidisciplinary team.
  • Demonstrated skills in data analysis, organization, and problem-solving.
  • Knowledge of issues impacting ECM populations, including homelessness, high utilizers, and individuals with serious mental illness (SMI) and/or substance use disorders (SUD).
  • Familiarity with HIPAA standards and compliance protocols.
  • Experience with Electronic Health Records; NextGen experience preferred
