The Resident Services Coordinator will provide residents with referrals, assist residents in accessing community resources, and empower residents by locating and/or initiating enrichment services on- and off-site. Community Resources:

<li>Identify the strengths of residents for asset-building and personal development as well as their needs for supportive services.</li>

<li>Conduct outreach and community engagement to promote program participation.</li>
<li>Set up office hours at each of buildings and advertise office hours for residents to meet with and request referrals.</li>
<li>Assist residents in building informal support networks with other residents, family members, and friends.</li>
<li>Create opportunities for residents to get involved with community activities.</li>
<li>Organize onsite community-building events.</li>
<li>Exchange resources with other Resident Services staff and collaborate on programs and community-building activities, as appropriate.</li>

<li>Accurately complete and submit timesheets by deadlines, reporting hours of work performed and allocating time on CAS report, monthly service reports, grant reports, activities budgets, program reporting, and any other required paperwork/reporting needed for the department.</li>

<li>Conduct surveys to assess resident needs, tabulating survey results to evaluate programs in order to plan relevant programs for the communities that you serve.</li>

<li>Responsible for maintaining a current and updated outlook calendar of activities at assigned sites.</li>

The attendant is directly responsible for personal care and support to meet the goals and objectives of the individual that they serve. The Direct Support Professional 1 (DSP 1) shall be aware that the consumers may terminate individual service. FUTURE TRANSITIONS, INC., is an At-Will employer, meaning that the employer or employee with or without cause, adhering to the law governing employee/employer relationships, may terminate employment at any time. The DSP will understand that Future Transitions Inc. has an obligation to the clients we serve and due to their vulnerability, the DSP’s employment is contingent on a successful completion of a background check, Live Scan and DMV report.
Essential Job Duties: 
1. Assist or perform for the consumer(s) in personal care, such as grooming, feeding, toileting, meal preparation. Assist or perform for consumer(s) basic housekeeping tasks, laundry and grocery shopping, and other personal care services as assigned by DSP 2.

One year of experience providing care to persons with developmental disabilities. Experience working with seniors in a human service field is also acceptable.

 c. Current vehicle registration and use of a vehicle in good running condition during shift hours.

What You’ll Do
As a DSP, you will support individuals with developmental disabilities in achieving the highest possible quality of life. You will promote their safety, well-being, independence, and personal goals while serving as their advocate.
Essential Responsibilities
