Pay Rate Range: $35-40/HR*

Position Overview:
The LVN provides and documents skilled nursing care under RN supervision, following a care plan and physician orders, while maintaining professional boundaries and confidentiality.
Key Responsibilities:
- Deliver quality patient care in compliance with physician orders and care plan
- Monitor patient conditions and communicate changes promptly
- Complete timely and accurate documentation
- Educate patients and families on care techniques and disease management
- Follow infection control and safety procedures
- Stay current with continuing education and required competencies
Requirements:
- Active, unrestricted CA LVN license
- CPR certification (hands-on)
- TB test – 2-step, within the last 12 months
- Minimum 1 year of LVN experience
- Reliable transportation
- Proficient in assessments, documentation, and care standards
- Continuing education as required by the state
Preferred:
- 6 months of recent clinical care experience
- Home health experience
