Title: Occupational Therapist – SNF
Location: Grass Valley, CA
Duration: 13 weeks (13–26 weeks preferred)
Shift: Day shift, 5x8-hour schedule (8:00 AM – 4:30 PM); occasional weekend coverage as needed

Job responsibilities and all required and preferred skills and requirements

Responsibilities:

  Provide occupational therapy services in a skilled nursing facility setting.

  \t

• \t

  Evaluate patients and develop individualized treatment plans to improve functional independence.

  \t

\t

• \t

  Deliver therapy focused on activities of daily living (ADLs), orthopedics, and pediatric/acute care populations as applicable.

  \t

\t

• \t

  Perform therapeutic modalities and manual therapy techniques.

  \t

\t

• \t

  Document all treatments and progress using required charting systems.

  \t

\t

• \t

  Collaborate with interdisciplinary care teams to support patient recovery and discharge planning.

Required Qualifications & Experience:

  Minimum 1 year of experience preferred in SNF specialty (new graduates will be considered).

  Active California OT license in hand at time of submission.

  \t

  Charting system experience required (any system accepted).

  \t
