Job DescriptionSalary: $25.00 per hour
Harvest Healthcare Solutions has immediate openings for Bilingual Patient Care Navigators (Spanish/English) to support clients enrolled in the CalAIM Enhanced Care Management (ECM) and Community Supports Program.
Patient Care Navigators play a critical role in helping clientsprimarily seniorsstay connected to medical care and essential community resources. This position works collaboratively with Care Coordinators, Community Health Workers, Program Directors, and healthcare providers to address barriers to care and improve health outcomes.
This is a part-time, community-based role that includes both telephonic support and in-field outreach throughout Placer or Yuba County.
Qualifications:
- Provide care navigation services to CalAIM ECM and Community Support members
- Build trusting, supportive relationships with clients to promote engagement in medical and social services
- Maintain timely and accurate documentation in compliance with program and state requirements
- Serve as a culturally responsive advocate for clients and their healthcare needs
- Communicates with Care Team members on a routine basis to support care delivery for patients.
- Identify and connect patients to resources for all clients to overcome barriers to care, such as transportation, housing, food, and other social service resources.
- Schedule and attend primary care physician appointments to review and update care plans with the Care Team.
- Ability to assess for and make appropriate referrals for any identified mental health or psychosocial problems.
- Ability to maintain client case records in a clear and concise manner in database.
Physical Requirements:
Physical requirements are those that are present in normal office environment conditions and will involve light to moderate lifting. Duties may involve occasional evening and weekend work.
Job Type: Part-time (20 hours per week)
Pay: $25.00 per hour

Education:
- Associates or bachelors in social or human services preferred
License/Certification:
- Candidates must possess a valid California drivers license and meet the States automobile insurability requirement. Candidates must be able to pass a DOJ criminal background check.
