Our Mobile Primary Care Team is seeking a dedicated and compassionate Physician Assistant or Nurse Practitioner with a heart for senior care and a passion for redefining how older adults and their caregivers experience health and well-being. This position blends independent practice and team-based collaboration, supported by a model that values compassion, connection, and clinical excellence.

We look forward to meeting you and putting a heart of GOLD back in the Golden Years!

At Brigade and its Affiliates, we're redefining traditional healthcare by bringing medical expertise directly to where seniors live – their homes and communities. The Brigade has grown to serve hundreds of senior living communities across multiple states, earning recognition for our commitment to personalized, relationship-based care.

As a Physician Assistant or Nurse Practitioner, you'll join a purpose-driven team that:

  • Delivers continuum-based care – integrating primary, psych, palliative, and home-based services.
  • Is built on a collaborative culture, where operational, clinical, and peer leaders support your long-term success.
  • Values loyalty and partnership with your pod and other Brigade leadership, fostering stability, mentorship, and growth.
  • Combines clinical excellence and efficiency with compassion, consistency, and connection.
  • Supports your autonomy through structured onboarding, coaching, and clear playbooks.

What Makes this Role Unique

  • Continuum-Based Care (Clinical Excellence): Deliver high-quality care across home, senior living, and community settings, supported by a multidisciplinary team in palliative, psych, and neurology.
  • Dementia Leadership: Build deep expertise in dementia-informed care and caregiver support as part of our expanding geri-psych and cognitive health initiative.
  • Autonomy with Support: Growth and quality are equally valued – encounter volume, patient retention, and outcomes all matter.
  • Collaborative Culture: Work alongside caring peers and leaders committed to your professional development and success.

Your Responsibilities Include:

  • Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
  • Delivering dementia-informed, relationship-based care across the continuum of care.
  • Conducting comprehensive assessments and developing tailored care plans for older adults.
  • Managing chronic conditions and performing care with skill and empathy for patients and their families.
  • Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.
  • Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
  • Maintaining consistency in documentation, follow-through, and patient communication.

Compensation and Benefits

  • Competitive Pay: Base Salary + Productivity (dependent on number of days worked).
  • Comprehensive Benefits: Health, dental, vision, and 401(k) coverage for your peace of mind.
  • Flexible Schedule: Enjoy stability and balance while making a measurable difference in patients' lives.

What You'll Bring

  • Active, unencumbered NP or PA license in the state you will be practicing.
  • Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
  • Compassionate, patient-centered approach and strong clinical judgment.
  • Valid driver's license and reliable transportation.

Endless Growth Opportunities: Join a dynamic team committed to your professional development, offering pathways for advancement and career enrichment within our expanding organization.
