Department: Foster Family Services
Location: On-site
Reports To: Program Director
Salary: $90,000–$100,000 annually + $3,000 Sign-On Bonus
Position Summary
The Lead Medical Coordinator serves as the primary liaison between healthcare providers and Foster Family Services to ensure clients receive comprehensive, timely, and trauma-informed medical care. This role oversees all aspects of client health coordination, including medical appointments, immunizations, medications, and ongoing healthcare needs, while ensuring compliance with California Community Care Licensing (Title 22), Department of Health, and County Public Health requirements. The Lead Medical Coordinator also supervises medical staff, maintains accurate medical documentation, and supports a safe, nurturing environment for all clients.
Essential Duties & Responsibilities
Coordinate team-based medical care for clients placed in the center
Prepare, maintain, and audit accurate medical records in compliance with agency policy, Title 22, licensing, and funding requirements
Review and update the ORR portal to ensure timely and compliant medical information uploads
Communicate medical updates to sponsors, family members, and clients as appropriate
Participate in discharge planning and medical resource development
Establish and maintain relationships with healthcare providers and community partners
Monitor medical assessments, document delays, and ensure follow-up
Represent client medical needs in meetings and interdisciplinary team discussions
Ensure adherence to child abuse reporting and incident reporting requirements
Supervise, coach, and mentor Medical Department staff, including performance management when needed
Attend required meetings and trainings
Safely operate company-issued vehicles to transport clients to appointments and activities
Maintain active medical licensure
Perform additional duties as assigned
Supervisory Responsibilities
Direct supervision of Medical Department staff
Provide leadership, guidance, training, and performance oversight in accordance with organizational policy and applicable laws
Qualifications
Required:
Valid driver’s license
2+ years of experience in child welfare
1+ year of experience in public health or a healthcare setting with demonstrated care coordination or clinical experience
One of the following credentials:
Licensed Registered Nurse (RN) with a Bachelor’s degree
Advanced health-related degree (Physician Assistant or Nurse Practitioner)
Preferred Qualifications
Experience working with children and families in foster care or residential settings
Bilingual in Spanish (preferred, not required)
Skills & Competencies
Strong care coordination and clinical judgment
Knowledge of healthcare compliance and regulatory standards
Excellent communication, organization, and problem-solving skills
Ability to lead teams and manage competing priorities
Proficiency in Microsoft Office and Electronic Medical Record (EMR) systems
Experience with UC Portal software a plus
