Job Description

This is temporary, the assignment has no end date. It is 20 hours/ week divided up over 4 days, M - F 9AM - 5PM. It is at 1 location.

Primary responsibilities are services coordination, advocacy, and providing linkages to adult residents. Provide in-take, assessment, and referral services to adults and youth needing services, in order to link them with and utilize existing community resources. Provide and/ or coordinate appropriate educational sessions for residents about resources available to them within the broader community. Monitor the ongoing services that are delivered to residents by community providers to assure that they are appropriate and that the resident is progressing. Ability to coordinate outreach, organize, and manage volunteers and vendors as needed to expand site programs provided; including but not limited to: food distribution, job readiness, nutrition, exercise, wellness, and other educational classes.

QUALIFICATIONS

BA Degree
3 years of experience in social services
