Position Overview
At Benevolence Health Care Center, we are committed to providing compassionate,
patient-centered care. The Patient Retention Coordinator plays a key role in building lasting relationships with patients, ensuring they feel supported, valued, and engaged in their care journey. This position focuses on improving patient satisfaction, reducing missed appointments and fostering long-term continuity of care.
Key Responsibilities
• Check patient eligibility on insurance portals.
• Schedule appointments to close missing HEDIS measures, Physicals, AWEs, and WCVs.
• Proactively reach out to patients to confirm and reschedule appointments, reducing no-shows and gaps in care.
• Conduct follow-up calls with patients who miss appointments or discontinue services, addressing barriers to returning.
• Educate patients about Benevolence Health Care Center’s services, preventive screenings, HEDIS care gaps and measures.
• Collaborate with clinical staff to coordinate patient needs, to improve quality of care, and increase organizational RAFT scores.
• Track patient engagement and retention trends using the EMR system and provide regular reports to management.
• Support outreach initiatives such as health campaigns, community events, and educational programs to keep patients engaged.
• Create and maintain new patient charts.
• Uphold HIPAA and confidentiality standards in all patient communications.
• Upload medical documents into patient charts using EMR (ECW).
• Additional responsibilities will be assigned once training is complete.
Qualifications
• High school diploma or equivalent required.
• Experience in healthcare, patient relations, or customer service is strongly preferred.
• Knowledge of Medicare, Medicaid, and HMO insurance plans.
• Knowledge of ICD-10 and CPT codes preferred.
• Knowledge of HEDIS and Quality Measures preferred.
• Preferred knowledge of Excel spreadsheets and Microsoft programs.
• Excellent communication, empathy, and problem-solving skills.
• Ability to manage multiple priorities while maintaining attention to detail.
• Proficiency with electronic medical record (EMR) systems (ECW).
• Proficient in English/Spanish.
