At Martin Luther King, Jr. Community Hospital, we are looking for a dedicated RN Certified Diabetes Care & Education Specialist to provide expert education and support to patients, families, and clinical staff in our ambulatory care settings. Position Overview
As an RN Certified Diabetes Care & Education Specialist, you will:
- Develop, review, and implement educational materials based on the latest clinical guidelines.
- Promote self-management strategies to help patients achieve their health goals.
- Collaborate with interdisciplinary teams to ensure consistent and effective diabetes care.
- Participate in quality improvement initiatives to enhance diabetes education across our organization.
Essential Duties and Responsibilities
Your role will include:
- Providing evidence-based diabetes education to patients and their families.
- Serving as a consultant/resource for clinical staff regarding diabetes education.
- Building positive relationships with nursing and clinical leadership teams.
- Evaluating and improving diabetes education programs based on feedback.
- Monitoring patient outcomes and readiness to learn.
- Conducting group presentations and educational programs as needed.
- Ensuring compliance with HIPAA standards regarding patient information.
Position RequirementsA. Education
- Associate's Degree in Nursing required; Bachelor's Degree preferred.
B. Qualifications/Experience
- Minimum of two (2) years of clinical nursing experience, with at least two (2) years in diabetes education.
- Current California Nursing license.
- Certification of Diabetes Care and Education Specialist (CDCES) required within 6 months of hire.
- Experience with Electronic Health Records (EHR) preferred.
C. Special Skills/Knowledge
- Bilingual skills (Spanish) preferred.
- Basic computer skills.
- Current Basic Cardiac Life Support (BCLS) certification.
