Job Description:
As an LVN Case Manager, you will be responsible for managing and coordinating the care of patients in their homes, ensuring that clinical care is provided effectively, safely, and in compliance with applicable regulations and policies.
Key Responsibilities:
- Provide skilled nursing care in accordance with the plan of care.
- Coordinate patient care with the RN Case Manager, physicians, and interdisciplinary team.
- Conduct in-home assessments and document observations accurately and thoroughly.
- Monitor patient progress and communicate changes in condition to the clinical team.
- Ensure compliance with agency policies, procedures, and home health regulations.
- Educate patients and families on disease processes, medications, and self-care techniques.
- Maintain accurate and timely clinical documentation.
Qualifications:
- Current LVN license in the state of CA
- Minimum of 2-3 years of nursing experience, preferably in home health.
- Knowledge of OASIS documentation is a plus.
- Strong clinical assessment and critical thinking skills.
- Reliable transportation and a valid drivers license.
- Excellent communication and organizational skills.
