Role Overview
Reporting to the Health & Wellness Director, the Resident Care Coordinator actively ensures excellence in personalized senior care (24/7). This role blends leadership, communication, and hands-on coordination to maintain quality of care, staff scheduling, and regulatory compliance.
Key Responsibilities

Develop and manage a 4-week rolling master care schedule—optimize coverage, minimize overtime or agency use.

Coordinate scheduling and third-party services (e.g., physicians, home health, hospice).

Maintain logs—appointment, communication, task sheets—aligning with state regulations.

Serve as primary liaison for residents’ needs: assess, monitor physical/mental changes, and escalate to HWD.

Onboard, train & supervise Care Partners and supervisory staff.

Step in as Care Partner or Med Tech as needed (within licensure).

Assist with new resident move-ins/orientations, community wellness activities, and marketing efforts.

Ensure departmental readiness for state surveys and audits.
What We’re Looking For

3–5 years of healthcare or senior care experience, including staff coordination or scheduling.

Experience with medication administration and care-giving (RCFE preferred).

Strong scheduling skills—able to manage multiple priorities in a dynamic environment.

Proficient in Microsoft Office and adaptable to new healthcare software.

Excellent communication skills—written, verbal, and for group facilitation.

Physically able to assist with resident transfer and move-ins (lifting up to 50 lbs independently, per job requirements).
Qualifications

High school diploma or equivalent (per state requirements).

Proven leadership in supervising, mentoring, and training healthcare staff.

Ability to work flexible hours, including holidays, weekends, and evenings.
