Korzen Health is collaborating with an acute care psychiatric facility located in Ventura, CA situated on 16 acres with ocean views.

Our highly structured and comprehensive programs provide the very best in mental health care. We have earned The Joint Commission Gold Seal of Approval for hospital accreditation. We offer specialized units for Adult and Adolescent stabilization and safety. In addition to crisis stabilization, we offer a Dual Diagnosis Program designed to help patients with a mental health diagnosis and chemical dependency.

We are seeking a full-time (32 hours a week) Primary Care Nurse Practitioner to our team. This provider will conduct H/P's within an inpatient behavioral healthcare setting.

Qualifications:

• Active CA Nurse Practitioner
• 2+ years clinical care experience as a Nurse Practitioner
• Experience working with and managing a population with challenges such as behavioral health and/or SUD conditions
• Experience developing and delivering educational programs to nurses and affiliated medical staff
• Care management and coordination experience

Responsibilities:

• In this role, the Health Guide (NP) will assess both behavioral and physical health conditions through a thorough history and physical examination
• The Health Guide (NP ) will be responsible for:
• Capturing conditions not yet diagnosed, including by ordering additional studies as indicated
• Making individualized recommendations and/or referrals for programs/care
• Training and advising Guides and Community Resources Guides on clinical issues and treatments
• Supporting Guide Team on care coordination
• Leading care management
• Performing medication reconciliation post-discharge and as needed

This is a full-time, 32 hour a week employed position. Medical Malpractice, PTO, Healthcare benefits, Sick Time, 401K included.
