About the Role:
Join Skilled Nursing Pharmacy as a PM Staff Pharmacist and be part of a dedicated team that enhances patient care in skilled nursing facilities. This role offers you the opportunity to make a meaningful impact in the lives of patients while working in a supportive and collaborative environment.
Scheduled Times:
1:00pm to 6:00pm
Responsibilities:
- Provide pharmaceutical care and medication management for skilled nursing patients.
- Review and verify medication orders for accuracy and appropriateness.
- Consult with healthcare professionals about medication therapy and patient outcomes.
- Ensure compliance with state and federal pharmacy regulations.
- Participate in interdisciplinary team meetings to optimize patient care.
- Educate nursing staff and patients on medication usage and safety.
- Maintain accurate and up-to-date patient medication records.
- Assist in training and mentoring pharmacy technicians and interns.
Requirements:
- PharmD degree from an accredited pharmacy school.
- Active California pharmacist license with no disciplinary actions.
- Minimum of 2 years of experience in a clinical pharmacy setting.
- Strong knowledge of pharmacotherapy and medication management.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Detail-oriented with strong organizational skills.
- Commitment to continuous professional development and learning.
