We are seeking a compassionate, organized Case Manager to join our residential treatment team. This role supports clients through individualized, trauma-informed case management while helping them navigate recovery, stability, and life transitions. The Case Manager works closely with clinical staff, leadership, families, and community partners to ensure coordinated, client-centered care.
This position maintains a small caseload (no more than 6 clients) to allow for meaningful engagement, strong advocacy, and quality outcomes. Case Managers also facilitate groups and actively participate in the treatment team.
Key Responsibilities
- Maintain a caseload of up to 6 clients and provide ongoing case management support
- Complete comprehensive assessments using ASAM dimensions and other relevant tools
- Develop individualized, action-oriented case management plans with weekly goals and milestones
- Coordinate internal and external services including housing, benefits, medical care, mental health, legal, and vocational resources
- Communicate with families and external stakeholders as appropriate to support continuity of care
- Facilitate case management and psychoeducational groups
- Participate in weekly treatment team meetings and program reviews
- Orient new clients to the program, expectations, and facility
- Monitor client progress and adjust plans as needed in collaboration with the client and treatment team
- Promote client strengths, self-advocacy, and engagement in the recovery process
- Assist with discharge planning, aftercare coordination, and exit planning
- Complete required documentation accurately and on time
- Maintain a safe, supportive environment for clients, staff, and visitors
Qualifications
- Certified or registered counselor in California through an approved certifying organization
- Experience working with individuals in substance use and/or mental health treatment settings
- Experience developing discharge and aftercare plans using community resources
- Strong communication skills, both written and verbal
- Ability to work collaboratively as part of a multidisciplinary team
- CPR and First Aid certification
- Narcan certification
- Current TB test

- Strong organization and time-management skills
- Ability to facilitate groups
- Knowledge of ASAM criteria and case management best practices
- Comfortable working with clients in crisis or transition
