Caregiver:
Under the direction and supervision of the Supervisor/Manager, the Caregiver delivers personal care services to patients. This role is vital in enhancing the quality of life for patients by providing essential care and support.
Duties / Responsibilities:

Provide personal care and escort assistance based on individual care plans, including grooming, dressing, peri-care, showers, bathing, toileting, housekeeping, meal preparation/feeding, and laundry.

The report observed changes in patients' conditions in the interdisciplinary team.

Demonstrate safe practices by using proper body mechanics, adhering to universal precautions, and maintaining environmental safety.

Interact professionally with patients, families, and interdisciplinary team members, encouraging participation in conversations and activities.

Perform other related duties as assigned.
Required Qualifications:
-
Minimum one year of experience working with the elderly or frail population.

Ability to work independently and as part of a team.

Flexibility to work in various environments, including homes and centers.

Positive can-do attitude, conscientiousness, compassion, and thoroughness.

Computer system literacy

Valid CA Drivers license

BLS Certification from the American Heart Association
-
Bilingual in Spanish, Cantonese, or any other language is highly preferred

Home Health Aide in the State of California highly preferred
Physical Requirements:

Ability to stoop, reach, stand, walk, push, pull, lift, grasp, talk, hear, and perform repetitive motions of the wrist and hands.

Must have visual acuity.

Ability to lift/move weight within individual physical capability, use appropriate lifting devices, or request assistance when needed.
