Job Requirements
• Prime CA Sexual Harassment Prevention Training (valid for 2 years)
• License/Certificate Verification (Primary Source Verification of professional license and/or certification, as required for the position, ran within 30 days of start, updated upon expiration)
• Prime Education Verification
• COVID-19 Vaccine (2 vaccines and booster required or declination completed)
• MMR (Proof of 2 Vaccines or Positive Titer showing immunity)
• Fit Test (Annual Respirator fit test)
• Hepatitis B (3 doses, if not titers are required)
• Influenza Vaccine (Flu Vaccine or declination required November 1st 2025 - March 31st 2026)
• TB Test (Within year of start date)
• Driver's License/State ID or Passport
• Varicella (Vaccination/titer showing immunity)
• Tdap (Declination not accepted in some states)
• Physical/Health Statement (Annually, must state clear to work)
• Prime OIG
• Prime Core Competencies Exam (Must score 80% or higher)
• Prime Skills Checklist
• Prime Color-blind test (Must be completed annually)
• Prime Criminal Background Check (National, within one year of start date)
• Prime Agency Attestation
• Prime SAM/EPLS (Must search "Excluded Individual")
• Prime Form I-9 (Employment eligibility verification)
• Prime Specialty Exam (Must score 80% or higher)
• DCI/BVP (Workplace Violence Prevention)
• Prime Drug Screening (minimum 10-panel test within 60 days of start, includes THC saliva test for CA locations)
