The Skilled Nurse - LPN/LVN plays a critical role in delivering high-quality patient care within various healthcare settings across the United States. This position involves providing direct nursing care under the supervision of registered nurses and physicians, ensuring patient comfort, safety, and well-being. The nurse will be responsible for monitoring patient health, administering medications, and assisting with medical procedures while maintaining accurate and detailed patient records. Collaboration with interdisciplinary healthcare teams is essential to develop and implement individualized care plans that meet patients' physical and emotional needs. Ultimately, the role aims to promote positive health outcomes and enhance the overall patient experience through compassionate and skilled nursing care.

Minimum Qualifications:

  • Valid and current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state of employment.
  • High school diploma or equivalent; completion of an accredited practical nursing program.
  • Basic Life Support (BLS) certification.
  • Demonstrated ability to perform nursing duties in compliance with state and federal healthcare regulations.
  • Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.

Preferred Qualifications:

  • Experience working in a clinical or long-term care setting.
  • Additional certifications such as IV therapy, wound care, or geriatric nursing.
  • Familiarity with electronic health record (EHR) systems.
  • Ability to speak a second language relevant to the patient population served.
  • Continuing education or specialized training in patient care or nursing best practices.

Responsibilities:

  • Provide direct patient care including monitoring vital signs, administering medications, and assisting with daily living activities.
  • Observe and report changes in patient conditions to registered nurses and physicians promptly.
  • Maintain accurate and detailed documentation of patient care, treatments, and progress.
  • Assist in the development and implementation of individualized care plans in collaboration with healthcare team members.
  • Educate patients and their families on health maintenance, disease prevention, and post-treatment care.
  • Ensure compliance with healthcare regulations, safety protocols, and infection control standards.
  • Support patients emotionally and physically, fostering a compassionate and respectful care environment.
