Personal Service Coordinator / Intensive Case Manager

Summary: Under the general supervision of the Program Manager provide high quality Intensive Case Management Services (ICMS) to clients eligible for permanent housing through the Department of Health Services’ (DHS) Housing for Health (HFH) and other programs. Provide a client-centered approach and provide excellent customer service that is sensitive to the challenges that people experiencing homelessness with a range of medical and behavioral health issues face as they move into and maintain permanent supportive housing. Employ a “whatever it takes” approach to assist clients in their transition from homelessness to permanent housing.

<li>Assist individuals at every stage of the housing stabilization process including flexible services to meet the individual needs of clients.</li>
<li>Outreach and engagement including processing referrals from Department of Health Services (DHS), Coordinated Entry System (CES) partners and other Housing for Health (HFH) partners, establish rapport with clients, provide a “screening in” philosophy, conduct a face to face screenings, obtain consents, and linkages to other needed providers and services.</li>

<li>Ensure clients have access to health, mental health, and substance use disorder services.</li>
<li>Assists clients with establishing a medical home and maintaining continuity with their medical home.</li>
<li>Assist clients with obtaining income and/or establishing benefits, with locating and securing employment and volunteer and/or educational activities, assist with basic needs, life skills, community participation, improving and/or maintaining daily independent living skills, social/leisure, personal hygiene skills, budgeting and money management, legal issues and making appropriate referrals to overcome barriers to accessing and obtaining permanent housing, and provide transportation as needed.</li>
<li>Provide housing location services (if scattered-site), educate clients on tenant rights and responsibilities.</li>
<li>Educate client on the appropriate use of crisis intervention services versus 911 emergency calls, etc.</li>
<li>Provide eviction prevention counseling; work with property management and HFH partner to help clients resolve issues that threaten their housing stability including tenant rights and responsibilities.</li>

Mendocino Community Health Clinic (MCHC) is currently seeking a Director of OBGYN Services to lead Care for Her, our women’s health department. We are a collaborative practice looking for a physician to lead our team and provide full-scope patient care. Our team includes four full-time, experienced OB/GYNs and a part-time OB/GYN, eight Certified Nurse Midwives, and three Women’s Health Nurse Practitioners. Care for Her patients are seen for regularly scheduled appointments in our outpatient clinic and for surgeries and deliveries at Adventist Health Ukiah Valley (8 minutes from our clinic). The practice encompasses all aspects of obstetrics and gynecology care. 
Our health center is currently delivering approximately 90% of all babies in our county, which is near 800 deliveries per year. Our experienced and talented CNM's are present in the hospital 24/7 and often oversee the non-complex labor and deliveries, with consultation by our OB/GYN on call. Our OB/GYN on-call supports emergencies of MCHC patients as well as unassigned patients and provides back-up for local FP/OB's. 
 
Director responsibilities include, but are not limited to: Clinical supervision, peer review, credentialing committee, scheduling oversight, participation in clinical leadership meetings, policy/protocols, personnel issues, and building collaborative internal and external relationships.
 
Total Estimated Compensation: $525,000. - $565,000.

Clients are offered services in a manner that encourages hope while promoting resiliency, growth and recovery. The VetWell Mental Health Therapist will provide a full range of trauma informed services including assessment, individualized treatment planning, therapeutic services, personal service coordination, rehabilitation services, integrated co-occurring mental health treatment, social skills development, integrated vocational services, supported education and access to support services. Under direction of the Vet Well Team Clinical Program Administrator, the Mental Health Therapist provides clinical consultation and subject expertise for the team and ensures that each team member’s observations and interactions with the clients are considered for individual treatment planning. Working knowledge of: assessment, diagnosis, prognosis, counseling, and psychotherapeutic treatment of marriage, family, and child relationship dysfunctions; methods of developing and implementing a treatment plan; human service delivery system; current trends in the field of mental health; and laws and regulations applicable to mental health. Ability to: perform effective counseling and psychotherapy with individuals, families, and groups; develop and maintain cooperative relationships with clients’ families, members of other professional disciplines, social service agencies, and co-workers; relate professionally with persons with serious mental disorders; identify and utilize community resources; prepare and present clear and concise oral and written reports.Must have experience working with persons with severe and persistent mental illness, co-occurring disorders, personality disorders/complex trauma and be comfortable treating those with acute and complex presentations/histories (multiple hospitalizations for mental health issues, forensic backgrounds). Must comply with a TB assessment (possibly a TB test if needed to confirm a negative tuberculosis result) and receipt of a criminal record clearance through fingerprinting. Must be able to successfully complete First Aid and CPR training.
