Our Home Care Assistants are a vital part of our team, providing aid to our participants in their homes as well as contributing to the care planning process as a member of our interdisciplinary team (IDT). Reporting to the Home Care Supervisor, the Home Care Assistant will provide safe delivery of personal care services to WelbeHealth participants in their home, as scheduled and directed.
This role is different becauseHome Care Assistantsat WelbeHealth:

<li>Work a stable, full-time schedule – unlike traditional home care roles, you will be assigned participants to fill a full workday, and will be re-assigned in cases of cancellations or unforeseen circumstances</li>

On the day-to-day, you will:
<ul>

Job Summary:
The Lead Care Manager (LCM) role involves developing personalized care plans, coordinating member services, and collaborating closely with members and families, as well as Primary Care Providers to ensure they receive necessary medical treatment and support. The LCM will consult with members to determine their needs, develop individualized action plans, and work with care teams to manage the member experience effectively. Providing emotional support, resolving administrative issues, and ensuring timely access to care are key aspects of the position. Compassion, healthcare knowledge, and exceptional customer service are essential qualities for assisting members in becoming self-sufficient in health. The LCM will work with a diverse population of members enrolled in the Enhanced Care Management program, which may involve one or multiple members from the population of focus section below. 
Responsibilities: • Interview members to assess medical and social determinant of healthcare gaps and provide education about their condition and medication, while developing individualized care plans. • Respond to member inquiries and concerns, ensuring adherence to hospital and legal requirements. • Collaborate with interdisciplinary teams, locate medical and social resources, and coordinate social service plans. • Maintain on-going contact with members, via telehealth and in-person visitation. • Advocate for members, consult with healthcare providers, arrange appointments and treatment plans, evaluate member progress, and assist with healthcare barriers. • Maintain empathy and professionalism while contacting members and families. • Supporting behavioral health coordination, Substance Abuse and Community Resources. • Perform additional duties as assigned.

 Education and Experience: • High school diploma or GED required. • Minimum of 1 year experience in case management, member care, customer service, call center, or member care required. • Valid California driver's license and valid vehicle insurance required. • MA certificate or medical terminology knowledge preferred.

Each community is managed with the philosophy that we are servants in their home, rather than them being guests in ours. The Assistant Resident Manager (ARM) contributes to the shaping of communities by providing operational continuity to the TRM/ TPM portfolio. The ARM is assigned to a respective community to cover for extended absences and works collaboratively with an onsite team, supervisory team and outside agencies to provide quality affordable housing. When providing coverage, the ARM is responsible for all operations including: rent collections, tenant relations, occupancy, financial reporting, regulatory agency compliance, and physical aspect of the community. In additional to these duties, the ARM supports the management structure by conducting proactive audits assigned by the Field Operations Administrator. Essential Job Duties

<li>Establishes a positive, productive and professional relationship with the onsite and supervisory team. </li>
<li>Works collaboratively with the Property Manager to ensure the community is operating within approved budget.</li>
<li>As directed by Property Manager, enforces rental policies as specified in the approved management plan.</li>

<li>As directed by Property Manager, ensures all recertification notices are prepared and distributed according to timeline specified by management. Ensures successful completion of 100% of all annual recertifications by project due date as specified in the OPPM.</li>

Responsibilities:
The primary focus of the Lead Staff Home Support Specialist is:
<ol>
<li>Supporting individuals in their homes and community by providing supervision, training, companionship, behavioral support, and other duties as deemed necessary by the person receiving support and that person’s circle of support. </li>
<li>Providing crisis support.</li>
<li>Ensuring the safety of persons receiving support, and other people, involved with Harbor SLS.</li>
<li>Providing customer service. </li>
</ol>
 
Duties:
<ol>
<li>Assists facilitator to create schedules and ensure shift coverage as needed. </li>
<li>Schedules monthly house meetings, creates agenda with team support, and ensures staff attendance. </li>
<li>Collects monthly paperwork from home and turns it into office by deadline.</li>
<li>Completes Monthly Service Reviews and Quarterly Safety Checklists.</li>
<li>Responsible for the review and upkeep of all household safety supplies. </li>
<li>Updates house binders at least once a month, ensuring all paperwork (office forms, data collection forms, medication information, etc.) is up to date and available. </li>
<li>Oversees and prompts completion of daily notes from direct support staff and sets an example for accurate and timely completion of notes. </li>
<li>Picks up medication, reviews for accuracy, creates and updates medication log as needed, at least monthly. </li>
<li>Posts monthly activity calendar, records all planned activities and meetings on house schedule, and posts any other Harbor correspondence. </li>
<li>Ensures the person receiving support’s safety in the home and community. </li>
<li>Assists persons receiving support in achieving personal, behavioral, educational, or vocational goals as identified in the Individual Service Plan. </li>
<li>Assists with some light household duties such as cleaning, cooking, and household management (may include yard upkeep) to maintain the household in accordance to the guidelines determined by the person receiving support and person receiving support’s circle of support. No significant amount of time should be designated to these tasks. </li>
<li>Provides necessary support for persons receiving support to meet personal needs.</li>
<li>Provides skills training when applicable. </li>
<li>Maintains current CPR/First Aid certification. </li>
<li>Participates in all mandatory training. </li>
<li>Responds to crises in accordance with Harbor’s policy and training. </li>
<li>Assists persons receiving support with any medical needs or appointments. </li>
<li>Provides supervision and support to persons receiving support in the home and community. </li>
<li>Communicates and coordinates services with the facilitator and Harbor’s management team. </li>
<li>Develops and maintains written documentation, as requested. </li>

Job Duty Allocation
<ol>
<li>No time in excess of 20% of any given shift shall be allocated towards completing housekeeping tasks and duties alone. </li>
<li>Activities that pertain to training new staff on housekeeping duties or supporting persons we support to complete housekeeping duties should not be factored into the 20% of time allocated towards housekeeping tasks.</li>
<li>If you feel that you are spending more than 20% of your shifts completing housekeeping tasks please contact your direct supervisor to resolve this matter.</li>
<li>Examples of housekeeping tasks may include: washing dishes, laundry, scrubbing or sweeping floors.</li>

Qualifications/Education:
A successful lead staff is reliable, energetic, flexible, capable of working both independently and as part of a team, and is a positive role model for Harbor SLS’s clientele. Qualifications include:

<li>Satisfactory oral and written communication, math, and computer skills. </li>
<li>A high school diploma or its equivalent. </li>
<li>A minimum of one year of experience with adults who have developmental disabilities (or similar experience) is strongly preferred. </li>
<li>The ability to maintain CPR/First Aid certification. </li>
<li>California driver’s license, a good driving record, and a registered and insured vehicle. </li>
<li>Department of Justice, or criminal background check. </li>
<li>Familiarity with SLS programs and persons receiving support is also beneficial. </li>

Experience/Requirements:

<li>Personal Assistance: Ability to assist the individual with activities of daily living including bathing, toileting, and other self-help skills. </li>
<li>Lifting/Physical Demands: Ability to lift 50 or more pounds frequently. </li>
<li>Challenging Behaviors: Ability and willingness to support individuals with challenging behavior. </li>
<li>Ability to read and interpret documents, write specific reports and correspondence, apply mathematical concepts to practical situations, solve complex problems, and interpret written and verbal instructions is required. </li>

Status: Non-exempt, hourly position. The position may be full time or part time. If regularly scheduled for 30 or more hours each week, the employee is full time and eligible for all applicable benefits. 
 
Work Schedule: Hours dependent on the person receiving support’s need. Staff must be available during business, evening, weekend, and holiday hours.
