POSITION SUMMARY:
The RN Clinical Care Coordinator is responsible for the clinical care coordination of ALWP clients under the Care Coordination Agency (CCA). The RN Care Coordinator assists waiver recipients in gaining access to waiver services, state plan services and other community resources. The RN Clinical Care Coordinator also acts as the clinical resource for the CCT Program.

QUALIFICATIONS:

<li>Knowledge of The State of California Assisted Living Waiver Care Coordination Agency Provider Handbook and RCFE Provider Handbook and the clinical requirement of the CCT and ALWP transition process.</li>

JOB SUMMARY A Physical Therapist provides professional services within the scope of physical therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families. QUALIFICATIONS Successful completion &amp; graduate of a PT Program approved by the Commission on Accreditation in PT education (CAPTE) or successor organizations of the CAPTE or an education program outside of the United States determined to be substantially equivalent to physical therapist entry level education in the US by a credentials eval organization approved by the American PT Association or an organization identified in 8 CFR 212.15(e) as it relates to physical therapists. A current, active license to practice Physical Therapy in CA. A minimum of two (2) years' experience as a Physical Therapist. Criminal Background check &amp; Current CPR certification.  RESPONSIBILITIES/ESSENTIAL FUNCTIONS Receives the intake referral information and prepares PT paperwork/tools necessary for the visit. Obtains all pertinent medical history from patient, family or significant others. Performs PT assessment visit and documents accordingly. Performs home safety check and environmental assessment of the patient's home environment. Assists the physician in evaluating the level of function. Helps develop an appropriate and effective PT Plan of Care (POC) to be submitted to the physician for approval and implementation. Functionally reassess the patient for on-going therapy. Determines medical necessity for other services. Evaluates the patient's ADL and iADL abilities. Develops and implements the HHA plan of care when HHA services are ordered and PT is the only skilled discipline. Supervises the HHA in accordance with state/federal requirements and documents the supervision without having to be directed to do so, when PT is the only skilled discipline. Effectively communicates ongoing with patient and family regarding the POC and progression of such. Effectively instructs patients and families in the safe use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, mechanical lifts etc). Effectively communicates with other disciplines in the case. Communication with the patient's physician (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities. Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests. Submits ALL OASIS assessments/documentation within 48 hours of the OASIS visit. Performs clinical record reviews (CRR) in collaboration with the Nursing Supervisor (Peer Review). Submits the CRR timely each month. Participates in the Performance Improvement Committee process as requested. Participates in staff meetings and in-services. Participates in the growth of the office by being a willing preceptor for newly hired, same discipline employees. Participates in the planning, operation and evaluation of the therapy services of the organization. The Detox/Medical Assistant will provide care and service recovery focused related to the safe and appropriate administration of medical treatment (including medications) as prescribed by the Health Care Practitioner. The Detox/Medical Assistant will be responsible to provide safe, effective, and efficient implementation of direct care in accordance with established policies, procedures, and standards of care. 
Responsibilities:
<ul>
<li>Aid in conducting new patient initial assessments and providing orientation to new patients</li>
<li>Assess and document patients’ needs and care, collaborating with other BHT staff to manage patient care</li>
<li>Provide medical care and assist in the welcoming, admission, and discharge process for residents</li>
<li>Transcribe physician orders accurately and in a timely manner</li>
<li>Monitor self-administration of medication and treatment to residents</li>
<li>Accurately assess degree of detoxification by utilizing methods such as CIWA and COWS to monitor for over sedation or medical emergencies</li>
<li>Support program staff with resident de-escalation and crisis management in a manner which fully supports the needs of the resident</li>
<li>Respond to emergencies and initiate action as outlined in company policies and procedures</li>
<li>Maintain communication with supervisor, Health Care Practitioner, and/or other key staff regarding resident care </li>
<li>Perform necessary duties for monitoring the health status of residents per company policies and procedures</li>
