Summary: 
The Care Partner (CNA) provides direct care to residents in our Assisted Living neighborhood, including activities of daily living, therapeutic activities programs, and restorative programs with a special emphasis on providing for the unique needs of residents with mild to severe dementia. Assistance may be total, partial, or stand by depending upon the physical and cognitive ability of the resident.
Duties and Responsibilities:

Key Responsibilities 
<ul>
<li>Provide individual and/or family treatment to children who are at high risk of abuse and/or neglect and children with severe emotional difficulties.</li>
<li>Provide individual/family in home counseling services to and/or office-based treatment to children and adolescents with a range of psychological, emotional and behavioral problems.</li>
<li>Provide case-management services which may include interfacing with community psychiatric hospitals, Department of Family and Child Services (DCFS), Department of Mental Health (DMH) school of officials, law enforcement and others as appropriate.</li>
<li>Maintain client charts in a responsible manner and meet all paperwork deadlines and requirements. Maintain productivity rate at required Agency Standards.</li>
<li>Comply with applicable professional licensing or certification requirements and ethical standards. </li>
</ul>
Qualifications 
<ul>
<li>Registered with the BBS as an AMFT, APCC, ACSW, LMFT, LCSW, and LPCC in the State of California.</li>
<li>Minimum one year experience working with multiple-issue families (e.g. unemployment, poverty, substance abuse, homelessness, domestic violence or abuse). </li>
<li>Strong interest in providing in- home, strength-based, culturally relevant counseling services. </li>
<li>Ability to function as part of a multi-disciplinary team.</li>
<li>Commitment to providing field-based services to multi-cultural population. </li>
<li>Fluency in Spanish high preferred.</li>
<li>Valid California Driver’s License.</li>

Job Description
The Associate Dental Director collaborates with the Chief Dental Officer to lead the dental department with responsibility for providing a broad range of quality dental services. The Associate Dental Director works closely with the Chief Dental Officer to assure that all program and regulatory guidelines are met. The Associate Dental Director provides professional supervision of dentists, dental hygienists, and dental business office managers delivering dental care. Responsibilities include monitoring of the quality of dental care, managing recruitment and retention, and planning and evaluating services.
 
Reports to: Chief Dental Officer 
To perform effectively in this position, the employee must have: 
1. A Graduate of an accredited medical school with a Doctor of Surgery (DDS) Degree or a Doctor of Dental Medicine (DMD) Degree.
2. Current license to practice dentistry in the State of California and Drug Enforcement Administration (DEA) number.
3. Five years experience in the practice of dentistry, with two years of supervisory experience at a community clinic, FQHC preferred
4. Must maintain a current Cardio-Pulmonary Resuscitation (CPR) certification, in compliance with corporate CPR policy.
5. Must demonstrate through written and/or oral testing, the knowledge and skills necessary to provide care appropriate to the age of the patients served and has knowledge of the principles of growth and development over the life span. He or she is able to identify and categorize each patient’s age specific grouping of needs, such as those for infant, adolescent, adult or geriatric patients.
6. Thorough working knowledge of business English, spelling, punctuation, and contemporary general office practices and procedures
7. Fluency in English (speak, read and write).
8. Fluency in Spanish (speak, translate, read and write) is preferred.
9. Excellent leadership skills.
10. Computer skills in Microsoft Office programs (Word, Excel, etc.), electronic health record systems, and database systems.
11. Effective oral communications skills including public speaking experience.
12. Demonstrated ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles.

POSITION SUMMARY
The Nurse Practitioner/Physician Assistant is responsible for the day-to-day management of patients in the outpatient Urgent Care setting. The Provider manages the health of patients by providing evaluation and management of acute and chronic illnesses/conditions. Assists in the development of and participation in health care services related to an Industrial Medicine/Work Comp program. The Provider is responsible for the coordination of care with specialists and appropriate ancillary services and shall work collaboratively with all members of the health care team.
