She has difficulty getting around, needs help with daily living activities, and requires transportation to medical appointments. Schedule Monday 10-6 Tuesday 10-6 Wednesday 10-6 Thursday 10-6 Friday 10-6 Start Date: ASAP End Date / Length of Position: Ongoing&nbsp; Responsibilities 
<ul>
<li>Assist with mobility, including use of walker and wheelchair</li>
<li>Provide safe transfers and lifting (in/out of bed and support as needed)</li>
<li>Transport client to medical appointments (10 miles or less)</li>
<li>Support with toileting; client currently experiencing increasing accidents</li>
<li>Assistance with showering and general hygiene</li>
<li>Help maintain a consistent meal schedule and oversee regular eating</li>
<li>Encourage healthy habits (family is trying to help her reduce/stop smoking-- currently has been cigarette free for several weeks!)</li>

<li>Record and report changes in resident’s condition to the supervisor including but not limited to changes in resident’s ability to perform activities of daily living, skin changes, bruising, etc.</li>
<li>Use approved charting criteria and procedures to record pertinent information in resident charts</li>
<li>Must be able to lift at least 50 pounds.</li>
