Position Description:
The Lead Care Manager works as part of a multi-disciplinary team, offering enhanced case management and other social services to adults who may have complex health conditions, are/have been homeless, and possibly have substance abuse issues. The team is responsible for the provision of enhanced case management and care coordination services which may include assessment and plan development, linkage to assistance with obtaining housing, benefits acquisition, linkage to behavioral health services such as substance abuse treatment, mental health services, linkage to primary care and assistance in any other area of life in which the individual needs help or support in order to successfully live in the community.
 
Essential Duties and Responsibilities: (includes the following; other duties may be assigned)

<li>Support individuals with active listening, demonstration of empathy and carry a message of hope and recovery.</li>
<li>Observe, assess, intervene and refer (within one’s scope of practice) in situations with clients regarding mental health symptoms, physical health symptoms and substance use issues.</li>
<li>Develop service/goal plans with clients and meet with the clients as needed to encourage, support and advocate for progress and assist with skill development. .</li>
<li>Work with a multi-disciplinary team to provide integrated psychosocial rehabilitative services to clients in the community and their homes, and attend meetings to ensure continuity of care for clients.</li>
<li>Provide positive public relations, community referrals, information and support by linking clients to needed resources; advocate with and on behalf of clients and significant support persons the clients have identified.</li>
<li>Accurately complete all necessary forms, reports and paperwork required by the program on a timely basis (e.g., progress notes, special incident reports, discharge summaries, health care billing, Avatar forms, HMIS).</li>
<li>Facilitate planning of social, cultural, educational, supportive and recreational group activities.</li>
<li>Shop for food and household supplies to meet program or client needs.</li>
<li>Ensure the office, house, rooms, units and yard are clean and fit for occupancy, where applicable. Provide and assist with cleaning as needed.</li>
<li>Act as a designated Mandated Reporter for the observed or suspected abuse and/or neglect of children, disabled individuals, dependent adults and elder/senior citizens. Mandated Reporter also includes “Duty to Warn” which includes the responsibility to notify authorities and the intended victim(s) of physical harm.</li>
<li>Transport clients as needed.</li>
<li>Ensure the protection and confidentiality of all Protected Health Information (PHI) by following all policies and procedures in the Agency HIPAA Manual.</li>

Education and/or Experience:

<li>Must meet the qualifications of a Mental Health Assistant III (MHA III). Individuals must have:</li>

<li>A minimum of four (4) years of full-time equivalent (FTE) direct care experience in the mental health field providing mental health services; OR</li>
<li>Two (2) years of FTE direct care experience in a mental health-related field providing mental health services and two years of education (60 semesters or 90 quarter units) with a minimum of 12 semesters (18 quarter units) in a mental health-related subject such as child development, social work, human behavior, rehabilitation, psychology or alcohol and drug counseling; OR</li>
<li>Two (2) years of FTE direct care experience in a mental health-related field providing direct mental health services and a certificate of completion from the County Core Skills Training. (MHRS highly desired).</li>

<li>Ability to work with individuals who have a mental health diagnosis. Experience is highly desirable</li>

A day in the life of a Facilitator: As a facilitator, you will start your day by meeting with the client at an agreed upon location.The facilitator will provide necessary one-on-one support to the client for that day's activity and/or event. If the client's goal is to register for school, the facilitator will assist in all aspects of signing the client up. If the client's goal is to improve their communication and socialization skills, the facilitator will provide support by having the client participate in the community. If the client's objective is employment, the facilitator will assist the client in building a resume, applying for a job, practicing mock interviews, and when the client is hired. The facilitator supports the client for at least 3 to 4 hours per session two to three times a week. Lastly, after the day's shift is over, the facilitator and the client will complete a progress note together and sign it. 
Job Specific Essential Duties 
<ul>
<li>Train, support, and monitor clients while in the community and vocational settings.</li>
<li>Participate in the development of a circle of support, to build relationships with family, friends, neighbors, and other community.</li>
<li>Develop individualized adaptive support materials as needed.</li>
<li>Develop and expand clients’ participation in integrated employment activities.</li>
<li>Assist with development and implementation of clients’ annual plan, goals, and objectives </li>
<li>Participate in clients’ annual Individualized Program Plan (IPP) meeting </li>
<li>Provide same day shift coverage as requested by supervisor.</li>

Minimum Experience and Qualifications
<ul>
<li>Education: High School Diploma/GED</li>
<li>Experience: 1 year working in social services, education, or with the developmentally disabled population preferred</li>
<li>Driving: 3 years driving experience </li>
