The Resident Services Coordinator (“RSC I”) is responsible for providing supportive service to elderly and physically challenged residents living at TELACU Residential Management (“TRM”) housing communities. RSC Level I to Level II requires evaluation of employee skill and performance levels by the Director of Resident Services Programs (DRSP) and Resident Services Supervisor (RSS). Due to the nature of the work assigned, the timeframe and transition will vary, and in some instances, an RSC Level I may be permanently allocated an RSC Level I.

Responsible for creating partnerships with social services partners and health care providers facilitating care management meetings to ensure situations are coordinated across partner organizations while the aggregate need of the resident is supported through appropriate services and helps with transition plans in collaboration with hospitals, assisted living, rehabilitation facilities, nursing homes and impacted families, as needed.
Essential Job Duties 
<ul>
<li>Serves as resident advocate when an issue arises that affects their daily lives or well-being.</li>
<li>Initiates 504 process for reasonable accommodations and modifications to address residents needs and manages process to ensure timeliness of completion. Works with the 504 Coordinator and maintenance staff, as needed based on approval or denial of accommodation by completing the process with a written notification to resident of the final resolution of 504 request.</li>
<li>Prepare a Supportive Services Plan. The plan must include: [1] activities and services that facilitate safely aging in place by helping participants proactively address their health and social support needs; [2] provide an ongoing benefit to maximize the impact of the funds; and [3] include the use of evidence-based programming (EBP) or evidence-informed programs.</li>
<li>Supports the Resident Services Supervisor in preparing and writing grants, proposals, applications, and reporting.</li>

Summary: 
The Resident Services Coordinator at Mission Housing Development Corporation will provide a vital role in engaging supportive housing residents and providing services to help them retain housing, develop and achieve attainable housing-focused goals. The Resident Services Coordinator also supports behavioral changes which reduces mental health symptoms and improve daily function. In addition, the role of the Resident Services Coordinator will be to facilitate linkage to community resources, 3rd party contracted partners for residents as needed and care navigation services. 
Essential Functions: 
Community Resources

<li>Identifies the strengths of residents for asset-building and personal development as well as their needs for supportive services.</li>
<li>Researches, identifies, and links residents with resources such as rental assistance, transportation, adult education, childcare, health services, and employment assistance. SSI, Medicare, food stamps, and prescription drug assistance, senior services and all pertinent services that would benefit our community/residents.</li>
<li>Establishes relationship with, maintains and also follows up with community service providers.</li>

ABOUT THE ROLE:
As a team member of the Stepping Stones Housing program, the Resident Coordinator provides essential services and support to transitional age youth (TAY) who live in our brand new collaborative partnership with Stepping Stones to create a unique interim housing program. This position is assigned to the Stepping Stones site and monitors site safety and youth services. The Resident Coordinator is responsible for general oversight of the property, cleanliness, safety, and maintenance. Additionally, the Resident Coordinator builds rapport with youth, provides advocacy, coordinates activities/workshops/events, and provides crisis intervention and conflict management. The resident coordinator works closely with their team and other SPY programs to provide the best services to our youth. The ideal candidate will have a strong understanding of the various factors that contribute to youth experiencing homelessness. Specific knowledge or experience related to youth pregnancy and parenting is ideal.
WHAT YOU’LL DO:

<li>Build relationships with youth from a trauma-informed and harm-reduction perspective</li>

The Resident Services Coordinator (“RSC I”) is responsible for providing supportive service to elderly and physically challenged residents living at TELACU Residential Management (“TRM”) housing communities. RSC Level I to Level II requires evaluation of employee skill and performance levels by the Director of Resident Services Programs (DRSP) and Resident Services Supervisor (RSS). Due to the nature of the work assigned, the timeframe and transition will vary, and in some instances, an RSC Level I may be permanently allocated an RSC Level I.
The RSC I, works with residents in a comprehensive and proactive manner to assess and identify resident needs and implements individual health-driven aging plans and educates residents on available resources. The position is an advocate for residents, manages the 504 reasonable accommodation process, and works on case management to prevent premature institutionalization to supports residents to safely age in place. 
Responsible for creating partnerships with social services partners and health care providers facilitating care management meetings to ensure situations are coordinated across partner organizations while the aggregate need of the resident is supported through appropriate services and helps with transition plans in collaboration with hospitals, assisted living, rehabilitation facilities, nursing homes and impacted families, as needed.
Essential Job Duties

<li>Serves as resident advocate when an issue arises that affects their daily lives or well-being.</li>
<li>Initiates 504 process for reasonable accommodations and modifications to address residents needs and manages process to ensure timeliness of completion. Works with the 504 Coordinator and maintenance staff, as needed based on approval or denial of accommodation by completing the process with a written notification to resident of the final resolution of 504 request.</li>
<li>Prepare a Supportive Services Plan. <li>Conduct room audits and ensure safety, cleanliness, and infection control protocols are followed.</li>
<li>Participate in staff scheduling and shift transition coordination.</li>
<li>Support compliance with CCLD regulations and complete incident reporting as required.</li>
<li>Collaborate with the Resident Services Specialist (RSS) and Medication Room Coordinator (MRC) to align care delivery with service plans.</li>
</ul>
Qualifications
<ul>
<li>High school diploma or equivalent required; relevant certifications preferred.</li>
<li>Must be 18 years or older.</li>
<li>Experience in assisted living, memory care, or elder care strongly preferred.</li>
<li>Prior supervisory or lead caregiver experience is desirable.</li>
<li>Strong written and verbal communication skills.</li>
<li>Familiarity with care regulations and charting systems (e.g., ALIS) a plus.</li>
<li>Current CPR and First Aid certification.</li>
<li>TB clearance, Health Screening (LIC 503), and Criminal Record Clearance (LIC 508) required.</li>
