Direct Support Professional Caregiver

Under the direction of ECM Program Manager and the Lead Coordinator, the lead care manager will outreach and enroll clients in enhanced care management. Will work with leadership, providers, and managed care plans to determine the needs of high acuity, vulnerable patients. Providing basic housing assistance, patient tailored intensive case-management, developing a care/service plan; provide linkages to medical, psychiatric, social, educational, and other services as needed. Will also work with the Community Supports Program staff to provide team-based, patient-centered care management for homeless and at-risk of homelessness patients. 
Care managers are to work with medical and/ or behavioral providers and patients to implement and perpetuate treatment and chronic disease self-management for patients enrolled in ECM within as well as outside of the clinic. Will develop and maintain Care Management Plan’s and review with the team for compliance. Assist with crises within their teams, and their caseload. Coordinate support services and reviews progress toward goals for patients. Completes scales and assessments as appropriate, needed and as directed by LC. Program Manager and or Director of ECM. Will also be responsible for billing as part of requirements per managed care plans and DHCS.

ABOUT THE ROLE:
The Resident Coordinator works out of the Lighthouse Transitional Living Program in West Los Angeles. Lighthouse TLP is a housing first low-barrier, high tolerance program providing housing and comprehensive services to youth ages 18-24 who are experiencing housing instability. The Resident Coordinator is responsible for oversight and safety of the property and residents. This includes, but is not limited to, monitoring and maintaining cleanliness of the site and living spaces, carrying out day-to-day safety protocols, responding and de-escalating crises, conflict management among residents, and promoting a holistic approach to the wellness of our residents. This position requires some driving and lifting. 
 
WHAT YOU’LL DO:

<li>Daily site and overall property inspections to maintain curbside appeal and resident safety.</li>
<li>Maintain inventory and storage of program supplies, including food, program supplies, cleaning supplies, etc. </li>
<li>Assist with program intakes and exits, including preparing intake packets, documentation of requirements, and conducting exit service evaluations.</li>
<li>Coordinate and assist with food and meal support in the program, including ordering and receiving Food Pantry donations and picking up food as needed.</li>
<li>Report all issues concerning resident health, safety, maintenance or other areas of concern in a timely manner and complete required documentation. </li>
